SLCF&R FireMed is changing

Effective July 1, South Lane Fire & Rescue (SLCF&R) Firemed will allow two uses for medically necessary ambulance transports per household member, per membership year. If the SLCF&R Firemed subscriber is transported to Cottage Grove Hospital by SLCF&R, and within a 24-hour period, is transferred to a higher level of care, as a result of the original condition, SLCF&R Firemed will allow membership benefit to be used and not counted toward the maximum use of two times per household member, per membership year.

SLCF&R Firemed will discount the bill 50 percent for subscribers without insurance coverage who are transported to a hospital. This benefit is available twice per household member, per membership year. If the SLCF&R Firemed subscriber is transported to Cottage Grove Hospital by SLCF&R, and within a 24-hour period, is transferred to a higher level of care, as a result of the original condition, SLCF&R Firemed will allow membership benefit to be used, and not counted toward the maximum use of two times per household member, per membership year.

SLCF&R Firemed allows one non-transport-related charge per household member, per membership year. Once the maximum of one non-transport-related charge is covered within a membership year for a household member, no further non-transport-related charges will be covered by SLCF&R FireMed for that household member, for the remainder of the membership year.

Firemed household membership benefits cover you, your spouse or domestic partner and dependents claimed on your income tax return that live in the same household. Elderly or disabled family members living in the same household are also covered.
